Combination Approaches in PD-L1–Positive mTNBC: Practice-Changing Frontline Evidence

Activity Overview

For most patients with immunotherapy-eligible metastatic triple-negative breast cancer (mTNBC) the frontline standard of care is chemotherapy plus pembrolizumab, based on results of the KEYNOTE-355 trial. Antibody-drug conjugates (ADCs) have been used in the second-line setting or later. However, recent results from a pivotal clinical trial challenged this paradigm, with the combination of a TROP2-directed ADC plus pembrolizumab demonstrating superior progression-free compared with a conventional chemotherapy-based combination. Ongoing trials are investigating other TROP2-directed ADCs with pembrolizumab in this setting.
