click to View Brochure and Agenda 2018 NEW YORK CARDIO-ENDO-RENAL COLLABORATIVE (NY CERC) focuses on a theme of Novel Approaches to Integrated Cardiometabolic Disease Management and will take place on September 29, 2018, in New York City at the Westin New York Grand Central. This unique, forward-thinking conference will feature innovation and advancements in novel therapies and technologies, patient engagement, lifestyle medicine, multidisciplinary approaches that are driving improvements in outcomes, and quality and cardiovascular/metabolic disease prevention. The 2018 NY CERC delivers state-of-the-art clinical updates and strategies aimed directly at the intersection of metabolic and cardiovascular disease (CVD) prevention.
 
We invite you to join us for this highly practical, fast-paced, and engaging conference that blends expert presentations on cutting-edge information with multiple “challenging cases” panel discussions to provide you with the latest clinical updates related to state-of-the-art treatment in patient care. Target Audience

This educational activity is directed toward cardiologists, endocrinologists, specialty nurse practitioners, specialty physician assistants, certified diabetes educators, other specialty allied health professionals, nephrologists, and other health care professionals (HCPs).

Learning Objectives

At the conclusion of this activity, the participant should be better prepared to:
 

  • Assess the challenges faced while treating complex cardiometabolic conditions, and determine approaches a clinical team can take to improve patient outcomes and reduce morbidity/mortality
  • Improve effective communication strategies focused on enhancing relationships between primary care providers and specialists, in order to restore adequacy in health outcomes of the cardiometabolic patient and population
  • Devise techniques that will enhance patient understanding of proper self-management of their cardiometabolic diseases by providing education about cardiovascular disease and diabetes prevention, adherence support for medication and other treatments, and tools and resources for self-management
  • Employ strategies that are both multifaceted and individualized geared toward attainable treatment goals and improved patient outcomes
