Overview

NEW YORK CARDIO-ENDO-RENAL COLLABORATIVE (NY CERC) focuses primarily on the strategic and novel approaches to management of integrated Cardiometabolic Diseases. This highly pragmatic and engaging conference blends in cutting-edge information delivered by expert academic physicians whom through a multitude of “challenging cases” provide the latest clinical updates related to state-of-the-art treatment in patient care. This conference will offer you the opportunity to advance your understanding and knowledge of chronic cardiometabolic and associated comorbidities.

Target Audience

This educational activity is directed toward cardiologists, endocrinologists, specialty nurse practitioners, specialty physician assistants, certified diabetes educators, other specialty allied health professionals, nephrologists, and other health care professionals (HCPs).

Learning Objectives

At the conclusion of this activity, the participant should be better prepared to:

  • Evaluate the challenges faced by clinicians while treating complex cardiometabolic conditions, and impose new methodologies in practice that will improve patient outcomes and reduce morbidity/mortality
  • Develop efficacious strategies focused on enhancing relationships between primary care providers and specialists, that will enhance health outcomes of the patient population experiencing these cardiometabolic disorders
  • Enhance patient knowledge and understanding of their cardiometabolic condition, by encompassing key strategies that emphasize the importance of self-management, prevention, adherence to medication and other treatments
  • Employ management approaches that are both multifaceted and individualized in order to improve treatment goals and patient outcomes
