New York Advanced Practice Collaborative Overview

We know that you are tasked with the management of a wide range of diseases, ailments, and symptoms. Whether it be diabetes or cardiovascular issues, when your patients walk into your practice, you treat it all. To help you stay up-to-date on the latest standards in care, this meeting blends expert presentations, panel discussions, and audience participation across a wide array of clinical topics. Join us to advance your understanding, knowledge, and competence in the prevention, treatment, and continued management of a multitude of chronic conditions, illnesses, and associated comorbidities. This new Advanced Practice Collaborative is as its name suggests, a gathering of the frontline practitioners dedicated to improving patient care, together.
Target Audience

This curriculum is directed toward Nurse Practitioners (NPs) and Physician Assistants (PAs) practicing in the primary care setting. Physicians and other healthcare professionals (HCPs) interested in the treatment of patients in the primary care setting may also attend and participate.

Learning Objectives:

Upon completion of this activity, participants should have improved their ability to:
  • DESCRIBE the collaborative role of all providers on the healthcare team
  • DISCUSS evidence-based guidelines and recent clinical study results that effect clinical decision-making
  • IMPLEMENT the latest diagnostic and treatment strategies for common conditions encountered in the primary care setting
  • DEVELOP methods to enhance the relationship between provider and patient to improve health outcomes
