Activity Overview

For patients with sickle cell disease (SCD), the transition from pediatric to adult care is a high-risk period that is often marked by gaps in care and worsening outcomes. This educational program brings together experienced hematologists to share data, clinical insights, and practical strategies for supporting adolescents and young adults through this critical phase. Faculty will discuss best practices for transition planning, common barriers to successful handoffs, and approaches to improving continuity of care and patient engagement. Through expert commentary and real-world examples, participants will gain actionable guidance to strengthen transition processes and improve long-term outcomes in SCD.

Target Audience

This educational activity is primarily directed toward hematologists, nurse practitioners, and physician assistants. Other clinicians involved in the care of SCD will also be invited to participate.
