Activity Overview
For patients with sickle cell disease (SCD), the transition from pediatric to adult care is a high-risk period that is often marked by gaps in care and worsening outcomes. This educational program brings together experienced hematologists to share data, clinical insights, and practical strategies for supporting adolescents and young adults through this critical phase. Faculty will discuss best practices for transition planning, common barriers to successful handoffs, and approaches to improving continuity of care and patient engagement. Target Audience
