New York Advanced Practice Collaborative Overview
We know that you are tasked with the management of a wide range of diseases, ailments, and symptoms. Whether it be diabetes or cardiovascular issues, when your patients walk into your practice, you treat it all. To help you stay up-to-date on the latest standards in care, this meeting blends expert presentations, panel discussions, and audience participation across a wide array of clinical topics. Join us to advance your understanding, knowledge, and competence in the prevention, treatment, and continued management of a multitude of chronic conditions, illnesses, and associated comorbidities. This new Advanced Practice Collaborative is as its name suggests, a gathering of the frontline practitioners dedicated to improving patient care, together.
This curriculum is directed toward Nurse Practitioners (NPs) and Physician Assistants (PAs) practicing in the primary care setting. Physicians and other healthcare professionals (HCPs) interested in the treatment of patients in the primary care setting may also attend and participate.
Upon completion of this activity, participants should have improved their ability to:
Review updates to guidelines and recommendations for screening and evaluation of patients presenting with illnesses commonly encountered in primary care
Discuss the potential impact of recent clinical trial data on clinical decision-making and care of patients in a variety of illnesses encountered in primary care
Identify strategies to recognize and manage treatment-related adverse events in primary care
Assess challenges faced by primary care practitioners in care of patients with a wide range of illnesses
Discern strategies for patient education, adherence, disease prevention, and self-management across a multidisciplinary team

A cancellation fee of 25% will be assessed on refunds requested before August 1, 2018, and a 50% fee on refunds requested from August 1, 2018, through August 24, 2018. No refunds will be made after August 25, 2018. There is no charge for substitution. Substitutions can only be applied to the same conference and only 2 substitutions will be honored.
