Spondyloarthritis (SpA) encompass a diverse group of chronic, inflammatory, and rheumatic diseases that share clinical, genetic, and pathophysiological characteristics, including involvement of the axial skeleton, peripheral manifestations, and extra-articular manifestations. Two major subgroups of SpA exist: axial SpA, which involves the sacroiliac joints and/or spine, and peripheral SpA, which is associated with peripheral manifestations such as arthritis, enthesitis, and dactylitis. Ankylosing spondylitis (AS) is an advanced form of axial SpA characterized by radiographic changes in the sacroiliac joints. Without proper treatment, AS can be a severe, debilitating disease, which can progress to cause structural damage of the axial or peripheral skeleton that leads to decreased mobility and quality of life. Delays in the diagnosis of AS have been attributed to the insidious development of the disease. Healthcare professionals often do not recognize the signs and symptoms of this condition, particularly during early stages prior to structural damage. The discussion will use a series of cases to foster discussion of new data and sharing of best practices that address unmet needs in AS.
