Physicians' Education Resource®, LLC is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.

Physicians' Education Resource®, LLC designates this live activity for a maximum of 25.0 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 25 MOC points in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider’s responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.

* E-syllabus materials will be available electronically prior to the meeting with updates to the slides presented available on site, and registrants will receive an active link with instructions on how to access the syllabus from our website for downloading and/or printing. It is suggested that you attend this event with your laptop and power will be available on-site.

A cancellation fee of 25% will be assessed on refunds requested through and including September 21, 2016, and a 50% fee on refunds requested on September 22, 2016, through October 19, 2016. No refunds will be made after October 19, 2016. There is no charge for substitution. Substitutions can only be applied to the same conference and only two substitutions will be honored.

In the event of a meeting cancellation, full refunds will be processed within 30 days of the cancellation date.
